Academic Forms and Routing Procedures
Forms: The academic forms for course, program, or policy implementation or change are as follows:
Form A: New Course Proposal Form - To be used for submitting proposal for a new course.
Form B: Course Change or Course Deletion Proposal Form - To be used to initiate a change in a present course or to delete a present course.
Form C: New Program or Track Proposal Form - To be used to initiate a new program or a new track in an existing program.
Form D: Program or Track Change or Deletion Proposal Form - To be used when an existing program or an existing track in a program is to be changed or deleted.
Form E: Academic Policy Request Form - To be used for all academic changes not involving academic courses or curricula.
